Actually, it's quite possible for a very popular house not to have any QAs, because they have so few women they invite to their preference round that there is less likelihood any of their attendees will be cross cut. At any rate, at Mizzou, the past two years 13 houses have had QAs, and in 2012 there were 11 houses with QAs. So it's extremely typical to have them at any houses that matches to quota. The only house that can't have a QA is one that doesn't match to quota, by definition.
