If you still make the grade cut, I wouldn't say anything. I don't know what your health concern was (nor should you specify it on this website), but you have to decide also whether you want everyone to know about it or not. If everything is fine now and it's all in the past, you might just want to wait a year before going through rush and use your college GPA if that's easier that putting it all out there. that also might give you a chance to really bounce back since freshman year can be a time of significant pressure.
