Not getting a bid after pref only happens if you attend two prefs and only put one on your card. That's called suiciding. If you are not high enough to match with that one house before they reach quota then you don't get a bid. But if you rank both your houses (or only the one if that's all you were invited to) then you are guaranteed a bid even if it puts a house over quota, which is called Quota Plus. You aren't guaranteed your first choice, but you are guaranteed a bid. TLDR: She either attended two prefs and suicided for theta, was dropped before pref, or she dropped out on her own.
