It's mostly a status thing. I remember sitting next to a pi phi in class a couple years ago while she was working on her professional resume. She had Cowboys sweetheart under her "awards and recognitions" section lol.

Frat sweethearts are picked by the guys. They are usually girlfriends or hang out a lot with the guys. Cowboy/Spurs sweethearts are picked by the current sweethearts and are usually their littles.
