rec letter question

What all should I include when sending information to people writing my rec letters? From what I've heard it's your resume, a cover letter, three pictures (head shot, full body, and half body), and an envelop with the address of the house the rec is going to. Is there anything else I should include?

Also include a copy of your transcript and a copy of your ACT or SAT scores.
